Check for any new signs/symptoms of PYELONEPHRITIS:

☐ Kidney pain/tenderness
☐ Flu like illness
☐ Shaking chills / temp 37.9°C+
☐ Nausea/vomiting
Urgent same day referral
• General practice
• Out of hours
Does the patient have ANY of the following:
☐ Vaginal discharge
☐ Urethritis
☐ Sexually transmitted infections risk
☐ Pregnancy signs/symptoms
☐ Genitourinary syndrome of menopause
☐ Immunosuppressed
Onward referral
• General practice
• Sexual health clinics

Does the patient have any of the 3 key diagnostic signs/symptoms

☐ Dysuria (burning pain when passing urine)
☐ New nocturia (needing to pass urine in the night)
☐ Urine cloudy to the naked eye
Are there other urinary symptoms:
☐ Urgency
☐ Frequency
☐ Visible haematuria
☐ Suprapubic pain
UTI less likely

Self-care and pain relief
Shared decision making approach using TARGET UTI resources
In patients that describe their symptoms as mild consider pain relief and self care as first line treatment.
Ask patient to return to Pharmacy if no improvement in 48 hours.
In patients with moderate to severe symptoms, offer nitrofurantoin for 3 days plus self-care.
